Ed Portillo, PharmD, is an Assistant Professor at the University of Wisconsin-Madison School of Pharmacy and a Clinical Pharmacist at the William S. Middleton Veterans Hospital. Dr. Portillo leads national deployment efforts of an interprofessional service titled COPD CARE, which has expanded to clinic locations across Illinois, Wisconsin, and Arkansas. As a Diffusion of Excellence Gold Status Fellow through the VA, Dr. Portillo coordinates educational programming for interprofessional colleagues to diffuse COPD CARE to VA practitioners from various disciplines. Dr. Portillo also is heavily engaged in teaching curricula focused on rural healthcare, leadership, and management to doctor of pharmacy students.

In clinical environments, learners often separate into profession specific settings, mirroring the unfortunate reality of a fragmented health care system. While health professionals assume responsibility for serving the same patients, they do so in separate spaces, often without the shared tools necessary to provide coordinated care.
