Jennifer Savage, DNP, APN, FNP-BC, Assistant Professor and Director, Family Nurse Practitioner (FNP) program, is a member of the Interprofessional Education Committee at Lincoln Memorial University. Dr. Savage has worked as an FNP at a Federally Qualified Community Health Center which provided mental health, dental, and primary care services in one building and nursing home rounds in which she was in communication with many healthcare disciplines to facilitate the coordination of patient care. She also owned a nurse practitioner ran clinic in a rural community. She currently practices for the Tennessee Department of Health as a nurse practitioner.

Research and practice wisdom demonstrate that health outcomes require more than simply prescribing the right medication for a disease. A wide variety of patient, family, caregiver, and community factors ranging from medication costs to high risk home situations may hinder desired outcomes and even lead to premature death. A pilot project was developed by health professions faculty from two schools to teach social determinants of health outcomes.
